- What were the reasons for each medium selected?
- What was the project budget, including estimate of staff hours, costs for outside consultant and in-kind contributions, frequency of distribution, and per piece cost, if applicable?
- Who is the target audience and what was the total audience served?
- Implementation
- How was the project developed?
- Did CAPIO members play a role in this project, if applicable?
- Were there any outside consultants?
- Results and evaluation
- How was the project evaluated and who was involved in the process?
- What were the evaluation results?
- How was the piece effective in targeting its audience?
- How were the goals and objectives met?
